A self-reflection worksheet

My Autism Profile

Rate eight areas from 1 to 5, and add a few notes on what each one looks like for you and what helps.

Tap a ring to set a level, or use the cards below.

How to rate each area

Each area has one statement, such as “I hide, adapt or perform parts of myself to fit in.” Rate how true that statement is for you, from 1 (not true for me) to 5 (very true for me).

A higher number means that area affects your everyday life more. It isn't a score of how autistic you are, and there are no right answers.

The scale

Think about a typical week, not your best or worst day. If an area changes a lot, pick a middle number and note what makes it change. Strengths belong in your notes too.

Tap any example that fits to add it to your notes. Write one point per line.

Your answers are saved in this browser, on this device only. Save a copy gives you a file, and Open a copy loads it back, here or on another device.

This is a reflection tool to use on your own or with a counsellor. It isn't a diagnostic assessment, and it doesn't measure whether someone is autistic.
